Most kids don’t like going to the doctor or dentist. When they go to the dentist, they are nervous because they are afraid of getting shots and being in pain. Putting off and staying away from the dentist because of fear can hurt a child’s oral health. Using conscious sedation techniques can help deal with this kind of fear in a safe way.
In dentistry, to sedate a patient means to calm him or her down, stop him or her from acting out, and make him or her quiet and cooperative so that the dental work can be done well.
To define: Sedation dentistry is a type of conscious sedation in which the patient’s level of consciousness is only slightly lowered. This means that the patient can still keep their own airway open and respond to physical and verbal commands.
At a consultation appointment before the delivery, the technique, sedative, and route of delivery should be chosen based on how well the patient fits with that technique.
Nitrous oxide, which is also called “laughing gas,” is the most common method of sedation because it is easy to use and safer than other methods. It has few side effects and is not very dangerous. It is done by giving the right amounts of nitrous oxide gas and oxygen through inhalation equipment.
For inhalation sedation to be safe and effective, you need to know everything there is to know about the different stages of pain relief and anaesthesia with N2O, as well as the delivery machine and circuits. This requires training in how to give the medicine and keep a close eye on the patient.
It is also important to know how to respond to emergencies and to be trained to do so. The equipment must be able to give out 100% oxygen, and it can’t give out less than 30% oxygen. If the reservoir bag doesn’t get bigger, look for a hole. Before using N2O to put someone to sleep, you should always carefully check the circuit and the equipment for leaks. Pulse oximetry should be required so that the patient’s pulse can be checked all the time.
On the day of the appointment, the child should eat normally and not drink alcohol. The child must take regular medicines as usual, unless the dentist or sedationist tells them not to. A responsible adult should be with the child at all times.
The dental chair is leaned back, and the mask is put close to the skin. The mask’s fit is checked. Patients can choose from a wide range of masks that smell good.
To start the procedure, 100% oxygen is given at a flow rate of 5–6L/min for 2–3 minutes. The reservoir bag is watched as the patient breathes. Each time the patient takes a breath in and out, the bag should move at the same rate. Pulse oximetry is a tool that can help with constant monitoring.
During the procedure, the eyes, general responses, and level of consciousness of the child are all watched. Nitrous oxide is measured every 10% up to a concentration of 30% and kept at that level. Slowly, local anaesthesia is given.
Once the procedure is done or almost done, the concentration of gas should be lowered so that the patient stays on 100% oxygen.
At least three to five minutes should be spent giving 100% oxygen. This gets rid of the nitrous oxide in the patient’s body and lowers the chance of post-procedure diffusion hypoxia, which happens when there isn’t enough oxygen.
Child will feel
Patients must be evaluated on an individual basis, taking into account their medical condition and how they respond. Child shouldn’t be able to leave home until he is fully recovered from being sedated. Give the parent clear instructions for what to do after the surgery. For the rest of the day, the child should sleep. The child shouldn’t do anything physical, and he or she should always be watched. Parents should be given written instructions for what to do after surgery.
